PERSONS AUTHORIZED TO FILE AN APPLICATION FOR TRAVEL CLEARANCE
- Either of the minor’s parent for legitimate minor
- Minor’s mother for illegitimate minor
- Travelling companion
- Legal Guardian (Court Order confirming guardianship should be provided)
EXEMPTED TO SECURE TRAVEL CLEARANCE
- A minor accompanied by the following:
- Either or both of the minor’s parents, if the minor is legitimate
- The minor’s biological mother, if the minor is illegitimate
- The minor’s father who has been granted sole parental authority or custody by the proper court, if the minor is illegitimate
- The minor’s legal guardian (Court Order confirming guardianship should be provided)
- The person, including one of the minor’s parents, who was granted sole parental authority or legal custody by the proper court, in which case the court decision should specifically include a statement to this effect, i.e., naming the person to whom sole parental authority of legal custody over the minor has been granted
- Minor’s adoptive parents if the minor is granted with adoption decree & Certificate of finality.
- A minor who is an immigrant or a permanent resident abroad, with foreign service holding a diplomatic/official passport such as Ambassador/General Consul/Attaches, provided he/she is holding a valid passport such as dependent visa/identification card which serves as a proof that the travel does not constitute child trafficking
CIRCUMSTANCES REQUIRING MANDATORY ISSUANCE OF CERTIFICATE OF EXEMPTION:
- If the parents are not married, and the child’s traveling companion is the biological father who has sole parental custody or legal custody over the minor as evidenced by a Court Order
- When the traveling companion is the legal guardian as evidenced by a Court Order
- Orphans of married parents and traveling with substitute parent/s grandparents or nearest kin per Family Code)
- Orphans of non-married parents and traveling with the substitute parent/s (maternal side)
REQUIREMENTS FOR SECURING THE MANDATORY CERTIFICATE OF EXEMPTION:
- QR-coded PSA issued a birth certificate of a minor;
- QR-coded PSA-issued Marriage Certificate of the minor’s parents
- QR-coded PSA-issued Death Certificate of minor’s parents
- QR-coded PSA-issued Birth Certificate of the traveling companion (grandparents or nearest kin) to establish the relationship with the minor
IF THE PARENTS ARE NOT MARRIED AND THE CHILD’S TRAVELING COMPANION IS THE BIOLOGICAL FATHER WHO HAS SOLE PARENTAL CUSTODY OR LEGAL CUSTODY OVER THE MINOR AS EVIDENCED BY A COURT ORDER:
- QR-Coded PSA-Issued Birth Certificate of the minor
- Scanned Court Order granting the Sole Parental Custody to the biological father
WHEN THE TRAVELING COMPANION IS THE LEGAL GUARDIAN:
- QR-Coded PSA-Issued Birth Certificate of the minor
- Scanned Court Order granting Legal Guardianship over the minor
ORPHANS OF NON-MARRIED PARENTS AND TRAVELING WITH SUBSTITUTE PARENT/S (MATERNAL SIDE):
- QR-coded PSA-issued birth certificate of a minor;
- QR-coded PSA-issued Death Certificate of the minor’s biological mother
- QR-coded PSA issued Birth Certificate of the traveling companion (maternal grandparents) to establish the relationship with the minor
BASIC REQUIREMENTS IN SECURING TRAVEL CLEARANCE CERTIFICATE:
- Duly accomplished Application Form (via online: https://mta.dswd.gov.ph)
- QR-coded Birth Certificate from the Philippine Statistics Authority (PSA)
- QR-coded Marriage Certificate of the minor’s parents or the Court Decision on Legal Guardianship, Solo Parent ID, if applicable;
- QR-coded Death Certificate of the deceased parent (if applicable)
- Notarized Affidavit of Consent allowing the minor to travel abroad, with the following conditions:
- If parents are giving consent to a legitimate child:
- A notarized affidavit of consent (notarized at the place of residence) signed by both parents, along with a copy of their valid ID(s) showing signature specimen.
- If mother is giving consent to an illegitimate child:
- A notarized affidavit of consent (notarized at the place of residence) signed by the mother, along with a copy of their valid ID(s) showing signature specimen.
- If the legal guardian is giving consent:
- A notarized affidavit of consent (notarized at the place of residence), plus a court order confirming guardianship, and a copy of the guardian’s valid ID with signature specimen.
- If the solo parent is giving consent:
- A notarized affidavit of consent (notarized at the place of residence), and the solo parent’s valid ID with signature specimen.
- If parents are giving consent to a legitimate child:
- Notarized Affidavit of Support and Certified copy of Evidence of financial capability of the sponsor such as any of the following: Certificate of Employment, Latest Income Tax Return, Bank Statement, etc.
- 2 original colored passport size photos of the minor (white background) of the minor taken within the last six (6) months from the time of application.
- Passport of the travelling companion with specimen
- Passport of the minor with specimen signature for over 13 years old.
- Notarized Affidavit of Undertaking by a Non Relative Traveling companion.
- Payment of P800.00 for issuance of Travel Clearance Certificate per minor.
ADDITIONAL REQUIREMENTS FOR MINORS UNDER SPECIAL CIRCUMSTANCES:
FOR A MINOR WHO IS OVER 18 YEARS OLD BUT IS UNABLE TO FULLY TAKE CARE OF HIMSELF/HERSELF FROM ABUSE, NEGLECT, CRUELTY, EXPLOITATION OR DISCRIMINATION BECAUSE OF PHYSICAL OR MENTAL DISABILITY OR CONDITION
- Certification from a Physician citing the minor’s condition and his/her inability to fully take care of him/herself.
For Minors who will attend Conference, Study Tours, Competition, Student Exchange Program, Summer Camp, Pilgrimage, World Youth Day and Other Related Activities
- Certification from Sponsoring Organization
- Notarized Affidavit of Undertaking of Companion indicating safety measures undertaken by the School, Sports Agency, or Organization
- Signed Invitation from the Sponsoring Agency/Organization abroad with itinerary of travel and list of participants and duration of the activity/travel
Minors going abroad for Medical Purposes
- Medical Abstract of the Minor
- Recommendation from the Attending Physician that such medical procedure is not available in the country
- Letter from the Sponsor
FOR MINORS UNDER ADOPTION (MUST HAVE AN UNDERGOING FILED PETITION)
- RACCO issued Consent to Travel
Minors under Legal Guardianship who will be unaccompanied by their Legal Guardian:
- Court Order on Legal Guardianship
For Minors with alleged missing parent/s
- Social Case Study Report from the LSWDO where the alleged missing parent’s last known address
- Blotter Report from either local police or Barangay Certification from the Locality of the last known address of the alleged missing parent (1 Original)
- Solo Parent ID, if applicable
